Medically Fragile Foster Care Program
NYAP offers Therapeutic Foster Care in a family setting to serve medically fragile children that require additional care and treatment. By definition, a medically fragile youth is one who has or is at risk of developing medical and/or developmental conditions that require ongoing, non-routine care.
A medically fragile youth may have one of the following needs or conditions:
- A need for durable medical equipment such as a wheel chair
- A condition that requires dependence upon support equipment such as a feeding pump
- A condition that is life threatening
- An acute, chronic infectious disease
- A condition that is a terminal illness
- A condition requiring on-going administration of medication (intravenous or otherwise
- A condition requiring intensive rehabilitation and/or developmental disability services
- A condition that results in an inability to carry out activities of daily living due to a medical condition

We work with our foster parents to ensure they have the knowledge, resources and support they need to care for the medically fragile youth placed in their home.
- Foster parents possess the skills and understanding of the roles and responsibilities required to care for these youth and are trained and fully licensed at foster parents.
- Foster parents ensure their home meets all required health standards based on the individual needs and standards required by the youth’s condition.
- NYAP meets with the foster family to make arrangements for a planned, paid respite for the foster family based on the medical condition and the services the family will have to provide.
Medically Fragile Youth, Ages Six and Under
For medically fragile youth, age six and under, NYAP works with foster families who are willing and able to take on the challenging and rewarding role of caring for more than one medically fragile youth (with a maximum of three youth at a time).
We work closely with these foster families to ensure they are supported with the resources, community supports, knowledge and skills necessary to care for the children and youth they foster.
