Care Management Program
Started in 1999, Care Management is a collaborative partnership between NYAP and Franklin County (Ohio) Children Services (FCCS). The Care Management Program is a flexible and creative continuum of care featuring practical approaches to resolving the challenges and crises youth and families face, day-to-day.
Holistic Approach. What makes this program so special and atypical is not only what we do, but how we do it. We work in partnership with families and parents to collectively do what is best for the child and the family as a whole. We go to great lengths to create service plans to suit the individual needs of the youth and/or family. We give families a voice and a choice in their therapeutic care and support.
Whatever It Takes. The program has innovative yet realistic approaches to service delivery with measurable success and positive outcomes. For one family, that meant keeping their entire family together and moving them to a NYAP owned home temporarily until the NYAP Maintenance Team could rehabilitate their almost condemned house. For another family, the solution was as simple as paying their utility bills to keep the electric and gas turned on in winter. For one youth, we arranged to have a ramp built outside of his home to make it easier for his parents to transport him for treatment. This reinforces our ongoing commitment to keep families together and to when at all possible, work with them in their own home, with supports and services from their own community.
The services available through the Care Management program are extensive and include 24/7 crisis and support, intensive home-based services, alcohol and drug services, parent skills training, links to community support services, residential services and much more.
We are proud to say that in 2008, 92% of youth and/or families involved in the Care Management program were provided services while the youth remained in their own home or the home of a relative with the goal of keeping the family together and personally involved in the youth’s care.
